What Nobody Is Telling You About GLP-1 Medications: The Risks of Using Them Without the Right Support
Let me be clear from the start: I am not here to vilify GLP-1 medications. I am here to give you the truth about them that most people are not hearing before they start, or while they are already on them.
GLP-1 receptor agonists — sold under names like Ozempic, Wegovy, Mounjaro, and Zepbound — have transformed conversations about weight, metabolism, and chronic disease. And there are genuinely powerful reasons for that. But the cultural narrative around these medications has become dangerously oversimplified. The message most people are getting is: take the shot, lose the weight, feel better. What is missing from that message is everything that needs to happen alongside the medication for it to actually support your health rather than quietly undermine it.
This is the conversation I want to have with you today.

What GLP-1 Medications Actually Do
GLP-1 stands for glucagon-like peptide-1. It is a hormone naturally produced in your gut that regulates blood sugar, slows gastric emptying, reduces appetite, and signals satiety to your brain. GLP-1 receptor agonists are medications that mimic and amplify this hormone's effects.
They work. For many people, they produce meaningful and sustained weight loss, improve insulin sensitivity, reduce blood sugar, and in some cases show remarkable effects on cardiovascular health and metabolic disease. These are not trivial outcomes.
But weight loss itself is not inherently the same as health. And how you lose weight matters enormously, particularly for women in perimenopause and menopause who are already navigating significant hormonal, metabolic, and structural changes in their bodies.
The Muscle Loss Problem
This is the issue I am most concerned about, and the one I believe is most under-discussed.
When people lose weight rapidly, whether through calorie restriction, bariatric surgery, or GLP-1 medications, a significant portion of that weight comes not from fat but from lean muscle mass. A systematic review of 22 clinical trials found that approximately 25 percent of total weight lost on GLP-1 medications consisted of lean mass. Other datasets, particularly when protein intake is very low, have shown this number climbing as high as 40 to 50 percent.
Dr. Gabrielle Lyon, founder of Muscle-Centric Medicine and one of the most important voices on the role of muscle in longevity and metabolic health, has been direct about this. Muscle loss on GLP-1 medications is not caused by the drug itself. It is caused by eating too little protein and not doing resistance training while the drug is suppressing appetite. The medication does not know the difference between fat and muscle. Your lifestyle choices determine how much of each you lose.
Why does this matter so much for women in perimenopause and menopause? Because you are already losing muscle. Estrogen plays a critical role in muscle protein synthesis and muscle preservation. As estrogen declines, muscle mass naturally decreases — a condition called sarcopenia — and your metabolism slows as a result. Adding rapid, unprotected weight loss on top of an already muscle-declining hormonal environment can accelerate this process dramatically, compressing what might take years into months.
Muscle is not vanity. Muscle is your metabolic engine. It is your insulin regulator. It is your bone protector. It is your longevity tissue. Losing it at an accelerated rate in midlife has consequences that extend far beyond how you look.
What is required to protect muscle on a GLP-1: a minimum of 30 grams of high-quality protein per meal, ideally more, and resistance training at least three times per week. These are not optional additions. They are the foundation without which GLP-1 therapy becomes a net negative for your long-term health.
The Bone Density Risk
Closely connected to muscle loss is bone loss, and this is another risk that is inadequately communicated to women starting GLP-1 therapy.
Bone density is maintained in part by mechanical loading — the pressure placed on bones by body weight and by muscle contraction during exercise. When weight drops rapidly, that mechanical stimulus decreases. When muscle mass is lost, the protective pull of muscle on bone decreases as well. And when caloric intake drops dramatically, as it does for many people on GLP-1 medications, the intake of calcium, vitamin D, magnesium, and other bone-critical nutrients often drops with it.
For women in perimenopause and menopause, who are already at elevated risk for osteopenia and osteoporosis due to declining estrogen's role in bone remodeling, this risk is not hypothetical. It is a real and measurable concern that requires intentional management.
Protecting bone health on a GLP-1 requires adequate protein, calcium-rich foods, vitamin D and K2 supplementation, resistance and weight-bearing exercise, and ideally bone density monitoring if you are in a higher-risk category.
Hair Loss: Why It Happens and What It Signals
A 2025 meta-analysis of over 84,000 GLP-1 users found that those taking these medications were 3.4 times more likely to experience hair loss compared to people not taking them. This is not a rare or unusual side effect. It is common, and it is worth understanding why it happens.
The primary driver is a condition called telogen effluvium — a stress-induced shift in the hair growth cycle that causes large numbers of hairs to enter the shedding phase simultaneously. The trigger is physiological stress, which rapid weight loss absolutely qualifies as. When the body is losing weight quickly and nutrient intake is restricted, it deprioritizes hair growth in favor of more critical functions.
Hair loss on a GLP-1 is your body's signal that the rate of change and the level of nutritional support are not in balance. It is not just cosmetic. It is a canary in the coal mine telling you that your nutritional foundation needs attention.
Key nutrients for hair health that are often depleted during GLP-1 use include iron and ferritin, zinc, biotin, protein, and omega-3 fatty acids. If you are on a GLP-1 and experiencing hair thinning, getting a full micronutrient panel and specifically checking ferritin is important. Dr. Jolene Brighten, who specializes in women's hormones and has written extensively on ferritin's underrecognized role in women's health, identifies low ferritin as one of the most common and most missed drivers of hair loss in women.
What Happens to Your Gut Microbiome
Last month I wrote about the estrobolome and the profound role your gut microbiome plays in hormone regulation, estrogen metabolism, serotonin production, and immune function. Everything I shared there becomes acutely relevant when we talk about GLP-1 therapy.
GLP-1 medications significantly suppress appetite. Many people on these medications are eating far less than they were before — sometimes dramatically less. While reducing caloric intake can be beneficial for metabolic health, the quality of what is being eaten and the diversity of the diet during this period matters enormously for the microbiome.
A healthy, diverse microbiome needs fiber, a wide variety of plant foods, fermented foods, and adequate calories to sustain the trillions of organisms doing its work. When caloric intake drops sharply and the focus shifts purely to eating less, the microbiome often suffers. Research published in 2025 confirms that GLP-1 therapies alter gut fermentation patterns and microbiome composition, with downstream effects on the short-chain fatty acids and metabolites that keep your gut and your hormones functioning well.
For women in perimenopause who are already experiencing microbiome disruption from declining estrogen, this compounds an existing vulnerability. The gut needs intentional support during GLP-1 therapy, not less attention.
GLP-1 Risks for Women: What Needs to Be Checked Before You Start
One of my strongest clinical concerns is how infrequently the appropriate foundational labs are run before a GLP-1 is prescribed. Here is what I believe every woman should have evaluated before beginning one of these medications:
Thyroid function — and not just TSH. A complete thyroid panel including Free T3, Free T4, Reverse T3, and thyroid antibodies. As I shared in my last blog post, an undertreated or undiagnosed thyroid condition dramatically affects metabolism, energy, and the body's ability to respond to weight loss interventions. Starting a GLP-1 on top of an unoptimized thyroid is layering one problem on top of another.
Iron and ferritin — ferritin in particular is often critically low in perimenopausal women and is rarely checked on standard panels. Low ferritin drives fatigue, hair loss, poor recovery, and impaired thyroid function. It needs to be identified and addressed before adding the metabolic stress of rapid weight loss.
Full hormone panel — estrogen, progesterone, testosterone, cortisol, DHEA. Your hormonal environment determines how your body responds to everything, including GLP-1 therapy. Optimization of hormones alongside metabolic support produces dramatically better outcomes than a medication in isolation.
Metabolic and inflammatory markers — fasting insulin, HbA1c, CRP, and a full lipid panel give a baseline picture of where your metabolism is starting and allow for meaningful tracking of progress.

Before Starting a GLP-1: Get These Labs Checked
Full Thyroid Panel — TSH, Free T3, Free T4, Reverse T3, and antibodies
Iron and Ferritin — low ferritin drives fatigue and hair loss
Full Hormone Panel — estrogen, progesterone, testosterone, cortisol, and DHEA
Metabolic Markers — fasting insulin, HbA1c, and full lipid panel
Inflammatory Markers — CRP and baseline inflammation
The Bottom Line for Part One
GLP-1 medications can be part of a genuinely powerful health strategy. I believe that and I will say it clearly. But they are a tool. One tool inside a much larger toolkit. And when used without the right nutritional foundation, the right movement practice, the right lab evaluation, and the right holistic support, they carry real risks that are not being adequately communicated to the women using them.
